Insurance Change Policy for Physicals

To Our Patients,

Due to the many policy changes in insurance plans, including Obamacare/Market Place Plans, effective 01/01/2016, diagnostic testing (EKG, Treadmill, DEXA, etc.) is not indicated by your insurance as preventive care or part of your annual physical examination. That also means diagnostic testing may be subject to your copay, deductible or coinsurance. While insurances have improved in the coverage of annual preventative care, they no longer consider any testing as preventive care.

Dr. Jasser, Dr. Lee, and Stephanie are committed to giving you the best medical care possible and will continue to do so, despite these changes. Please note, our providers will not change their current standard recommendations for you to have your necessary cardiac testing, bone density screening, or any testing for other chronic medical conditions that are necessary for your healthcare.

Insurances Accepted
Current Insurances:  Commercial and Medicare Advantage Plans.

BCBS, Humana, Cigna, Aetna, United Healthcare, Banner Health Network, Tricare, BCBS VAPC3, Medicare, Health Net Access, Care First, AARP Dual Complete, Health Net

If you have an HMO and need to have a PCP, please be sure that Dr. Jasser or Dr. Lee is the PCP listed on your insurance card or we cannot see you. If your insurance company has assigned you to another provider, you will just need to call member services to make the change.
